Upon arrival, please inform your server or manager of your chosen Party Package. To ensure a smooth experience, at least one member of your party should arrive within 15 minutes of your reservation. Late arrivals may result in releasing the table. No refunds will be given. You may bring freestanding or floating decorations, but confetti, glitter, and similar items are prohibited and may incur cleanup fees. Pricing excludes tax and gratuity. Per Maricopa County Health Codes, outside food and drinks are prohibited. A $100 deposit is required to secure your reservation, non-refundable and applied to your final bill. Deposits are non-refundable. The Forum may cancel reservations for event buyouts, with full deposit refunds processed.

Always a great place for live music, appetizers and drinks. Staff are friendly … food is sub par. Ordered chips and salsa and salsa was off. It took awhile before a server came to our table to take food and drink order … once order was placed, food and drinks arrived promptly. Had to flag server down several times to place additional drink orders and ask for water glasses to be refilled. Same issue when ready to cash out.

The vibe is off… food was pricey for the quantity. I imagine it would be a great place for private events, but for a Saturday night out… we got to listen to Gospel Christmas songs playing, and microphone checks for an hour. We left immediately after we finished eating. Between the Gospel Christmas songs and the lead singer preaching about being our best selves… I felt like I was in church with adult beverages. Vibe is off.

First time visiting and I like the vibe in the place. Friendly staff and great music. Being a food critic I love sneaking in on these places and being blown away by the food. For bar food, they did not disappoint A+
